Cleanse Clinic is a CARF Accredited outpatient behavioral health and addiction treatment facility. We strive to offer a premium service in the community by providing a comprehensive suite of integrated services that include intensive outpatient programs, psychiatric treatment, case management, medication-assisted treatment, individual and group counseling. Together these programs target all aspects of substance abuse and other mental health issues.

Job Title: Case Manager

Reports To: Clinical Supervisor

Job Description:

Case Managers provide additional support to patients active in our treatment programs, empowering patients by connecting them with additional resources or attention to improve their recovery. The role requires excellent verbal and written communication skills, self-sufficiency with independent judgment for prioritizing and completing tasks, and the ability to succeed in a teamwork environment to deliver patient care. This is a flexible role and may include administrative, supervisory, clinical, or other responsibilities.

Duties and Responsibilities may include:

Core Services

  • Document patient history with specific skill deficits, basic support needs, levels of functioning, risk status, define the scope and focus of the service plan
  • Assess and prioritize the needs and strengths of the population served
  • Collect information from medical professions, educators, social workers, and other sources including but not limited to family members, friends, and clergy
  • Create care plans that identify the mission and vision of the client as well as targets specify goals and objectives on how to obtain
  • Document progress notes with details and description of services provided and involvement & performance towards care plan goals
  • Coordination resources and refer to organizations in the community
  • Document discharge summary, significant findings, and condition at termination
  • Coordinate aftercare as the patient moves through the recovery process

Record Keeping

  • Document direct and indirect contacts with the patient
  • Accurately and timely maintain a face sheet with dates and times for completed documentation
  • Obtain prior medical records from physicians, treatment providers

Administrative

  • Obtain PA authorizations through third party providers
  • Organize, monitor, and obtain PA reauthorization
  • Monitor active caseload

Team Member

  • Communicate effectively pertinent information in cases with clinical staff
  • Monitor care plans, progress notes, and review of activities and contacts to obtain goals quarterly with clinical services supervisor
  • Attend care plan sessions

Qualifications

  • Bachelor’s degree in behavioral health, public health, nursing, education, occupational therapy, physical therapy and/or speech-language pathology AND 1-year full-time employment working directly with individuals in a human service setting
  • OR Masters Degree in behavioral science
  • Targeted Case Managment training preferred
  • Successful completion of Criminal Background check
