CHI Saint Joseph Health is seeking an acute care experienced Advanced Practice Provider to join our team in the role of Market Director of Access and Patient Logistics.  

Job Summary / Purpose

Provides leadership and strategic direction of capacity management, patient logistics, and patient flow operations.  Develops, implements, and operates a Central Command Center to coordinate all admissions, internal transfers, and discharges to ensure smooth patient access and patient flow operations. Coordinates the usage and functionality of the Patient Logistics applications with staff, IT, and vendor; applications include but are not limited to EPIC, TeleTracking, Clinical Apps, Cisco, Finesse, Calabrio, TigerConnect.  

Essential Key Job Responsibilities

  • Designs, Develops, implements, and operates a Central Command Center, including but not limited to Patient Placement, Transfer Center, and virtual care to coordinate all admissions, internal transfers and discharges, to ensure smooth patient access and patient flow operations for multiple facilities
  • Collaborates with leadership teams to promote innovation and drive continuous improvement of capacity management and patient logistics
  • Coordinates with external service providers to ensure compliance with established contract provisions
  • Coordinates the usage and functionality of applications with staff IT and vendors. Creates, updates, and validates all command center work aids, algorithms, and work processes to ensure accurate response for each type of case
  • Designs, Develops, and presents/shares performance reports and dashboards to demonstrate operational functions and identify areas of opportunity and best practice
  • Present to hospital executive teams data, performance information at patient flow and other hospital meetings; identifies areas for improvement and/or best practice
  • Recognize, accept, and cooperate with direction from system, division, market leadership
  • Ensure continuous competency of staff
  • Maintain awareness of issues related to nursing profession
  • Provide measurable written goals appropriate to level of experience and expected job duties during annual performance reviews
  • Participate in service line growth and quality initiatives

Qualifications

Minimum Qualifications

Required Education and Experience 

Acute Care Nurse Practitioner with Master’s or Doctorate Degree in Nursing required.

Master’s Degree Business Administration, Healthcare Administration, Nursing, or related field preferred.  Bachelor’s Degree in Business Administration, Healthcare Administration, Nursing, or related field required.

Minimum of five (5) years of progressive leadership in healthcare management

Must have knowledge of strategic planning, administrative and financial principles, and clinical practices.

Must have knowledge of hospital operations; staff scheduling, relevant experience leading a patient placement, bed management, transfer center or similar unit is preferred

Required Licensure and Certifications

Current Kentucky Nursing License or other State Nursing License with ability to obtain KY Nursing License within 6-months

Required Minimum Knowledge, Skills, Abilities and Training

Experience with TeleTracking or other bed management systems. Knowledge of hospital capacity management, patient logistics, and patient flow practices.  Excellent interpersonal; communication; leadership; and motivation skills to create high performance teams comprised of clinical, administrative, and service staff.  Positive customer service orientation.  Independent judgment to carry out major functional accountabilities and ability to complete job duties autonomously, responsible for appraising the performance of employees, hiring/firing, etc.  Strong experience with data management, including Excel, pivot tables, graphs and charts.  Experience working with healthcare information technology, such as electronic health record and patient flow tracking systems.  Ability to work in a fast-paced environment with rapidly changing priorities.  Requires strong work ethic and attention to detail.  Ability to interact with all levels of the organization, including physicians, senior management and community partners.  Proven leadership and supervisory skills.  Knowledge of process improvement techniques.
