Value Based Transitional Care Coordinator LPN

External DescriptionPOSITION SUMMARY: The Transitional Care Coordinator assists the Regional VP of Operations, local Branch andState Directors and SVP of Payer Innovations to implement, oversee and support Value Based products andproduct changes throughout a defined geography.

In this role, the Transitional Care Coordinator will clinicallymanage clients enrolled in value based programs and works closely with all involved departments including localoperations, the Health Engagement team, Support Center and Corporate personnel.

The Transitional CareCoordinator will work as a facilitator between the various internal departments to ensure product policy andprocesses are documented, and executed accurately and will serve as the front line clinical support includingperformance of home visits.

The Transitional Care Coordinator will also be responsible for collecting, processingand reporting data to internal and external customers.

The Transitional Care Coordinator will identifyrequirements for product implementation and will be responsible for pre-empting potential problems and workingwith the team to develop effective solutions.ESSENTIAL DUTIES:
•Receive incoming calls/referrals for clients enrolling in value based programs and maintain accurate listsof enrolled clients
•Coordinate or perform an initial in home visit with enrolled clients
•Ensure completion of a medication reconciliation for all enrolled clients
•Perform regular check in calls to clients regarding care plans and scheduled follow up
•Perform telephonic or in person training to clients on their medical conditions, medications and care plans
•Work with client to identify gaps in in-home care and refer client for home health, hospice or in homephysician services as needed.
•Identify additional local resources to assist client with clinical and social issues such as local paramedicalservices, urgent care centers, non-emergent transportation, pharmacy resources, etc.

Developrelationships with extended services and refer clients as necessary
•Prepare and deliver regular summary communication with clients Primary Care Provider
•Receive and document Change in Condition calls from the health engagement team and direct care staff
•Review and verify Change in Condition reports from Electronic Visit Verification and other electronicsystems, perform follow up as necessary
•Evaluate Change in Condition information for additional follow up
•Coordinate and build strong working relations with various internal departments
•Work with corporate resources to participate in the development and implementation of specializedtraining on new products launched and raise awareness of programs with local operations and direct carestaff
•Monitor internal processes for efficiency and validity pre post product launch/changes.
•Manage data flow from operations and report outcomes to internal and external customersPERFORMANCE RESPONSIBILITIES:

•Able to perform and manage multiple tasks and projects simultaneously
•Strong proficiency with Word, Excel, PowerPoint, and Outlook
•Able to widely distribute, compile, track, and monitor large amounts of data in a clear and concise manner.
•Proficient in multiple software programsPOSITION REQUIREMENTS COMPETENCIES:
•Current RN or LPN license with at least three (3) years of experience in a clinical setting
•Previous Experience in home care and home health.

Knowledge of managed home healthcare is preferred
•Candidate must possess excellent verbal and written skills
•Solid computer proficiency in Microsoft Office including Excel and web based application
•Committed to exceptional customer service and meeting customer needs
•Ability to complete tasks in a multi-phased, fast-paced environment
•Must undergo pre-hire drug screening
