Job Description Summary:

The LPN/LVN Nursing Clinical Coordinator is a qualified, licensed professional who is responsible to assist the Nurse Manager and Operations leader in the compliance of patients with prescribed drug therapies in accordance with the Option Care policies, Federal, State, professional standards of practice and accrediting body requirements.

Job Description:

Job Responsibilities:

  • Supports daily activities with regards to effective agency operations and communications.
  • Collaborates with Care Transition team members, Intake and Nursing on the onboarding of new referrals.
  • Supports the implementation of the Care Plan, Plan of Care and medication profile for new admissions and recertifications to include optimizing the use of tools used for this purpose. (NursePoint, PharmaPoint, ReferencePoint).
  • Manages daily activities with regards to effective operations and communications and ensures the office remains in compliance with all regulatory and accreditation entities within the scope of the position. Complies with reporting requirements, ensures policy and procedure compliance and monitors and evaluates medical supply usage.
  • Participates in the process for assessing, managing, and reporting nursing operational data to the Nurse Manager and or Director of Operations utilizing company scheduling, documentation, and dispensing applications to support these efforts.
  • Speaks knowledgeably of the scope of services that are available through Option Care. Description of available services is accurate.
  • Assists with the coordination of all agency activities related to nursing including direct and indirect patient services, scheduling, subcontract nursing communication, visit billing, and oversight of both patient scheduling needs and staff availability. Appropriately escalates any potential unmet staffing needs or requirements.
  • Maintains clinical competency by providing necessary patient care at least 30% of the time.
  • Assists in the implementation and evaluation of continuing education, orientation and clinical educational programs.
  • Participates in Infection Control and process improvement activities, including chart audits and implementation of identified Quality Improvement Plans.
  • Promptly identifies and supports the management of patient concerns and complaints and facilities their resolution, escalating when necessary to the appropriate department.
  • Maintains confidentiality of patient and proprietary information.
  • Facilitates and attends required in-services. Presents and/or attends all Option Care required in-services. Obtains all educational requirements necessary to maintain professional license and certifications.
  • Participates in the nursing department on-call responsibilities for appropriate phone triage and patient visits, as required.
  • Other duties as assigned.

Supervisory Responsibilities:

Does this position have supervisory responsibilities? No.
(i.e. hiring, recommending/approving promotions and pay increases, scheduling, performance reviews, discipline, etc.)

Basic Education and/or Experience Requirements

  • LPN or LVN with a license in the current state of practice and at least 2 years of home health care, infusion center, or medical office practice experience providing specialty nursing care, such as infusion therapy, maternal-child health, disease state management pediatric nursing, etc.
  • Required licensure in Nursing as granted by current state of practice.
  • Current CPR certification.

Basic Qualifications & Interests:

  • Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction.
  • Experience building and maintaining relationships within a team.
  • Experience using time management skills such as prioritizing/organizing and tracking details and meeting deadlines of multiple projects with varying completion dates.
  • Basic email skills, internet explorer (sending, receiving, and organizing communications).
  • Basic level skill in Microsoft Word (for example: opening a document, cutting, pasting and aligning text, selecting font type and size, changing margins and column width, sorting, inserting bullets, pictures and dates, using find and replace, undo, spell check, track changes, review pane and/or print functions).

Travel Requirements:

No travel required.

Preferred Qualifications & Interests:

  • Required Satisfactory completion of Infusion Therapy course work.
  • At least 3 years of previous infusion experience.
  • At least 5 years of previous Home Health Care experience.
  • Basic level skill in Microsoft Word (for example: opening a document, cutting, pasting and aligning text, selecting font type and size, changing margins and column width, sorting, inserting bullets, pictures and dates, using find and replace, undo, spell check, track changes, review pane and/or print functions).
  • Basic level skill in Microsoft Excel (for example: opening a workbook, inserting a row, selecting font style and size, formatting cells as currency, using copy, paste and save functions, aligning text, selecting cells, renaming a worksheet, inserting a column, selecting a chart style, inserting a worksheet, setting margins, selecting page orientation, using spell check and/or printing worksheets).
  • Basic skill level in Microsoft PowerPoint (for example: inserting, rearranging, hiding and deleting slides, navigating between slides, increasing list level, adding, centering and editing text, changing views, inserting a table or a note, moving objects, printing outline view and/or running a slide show).
