Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services.

Responsibilities: ESSENTIAL DUTIES AND RESPONSIBILITIES:Effectively utilizes the nursing process to manage patients with chronic disease within the Palliative Care Clinic Assumes responsibility and accountability for care plan effectiveness and client outcome.

Assesses medication adherence when patient specific goals are not met.

Based on collaboration with patient and assessment, defines nursing diagnosis and plan of care or communicates with provider for medical advice and plan of care.

Implements care plan.

Utilizes protocols orders as appropriate.

Qualifications: Education: Bachelor’s Degree in nursing preferred.

Licensure: Currently licensed with the applicable state nursing board and/or possess multistate licensure privileges.

Functions within the legal scope of practice.

Must hold a valid drivers license.

Experience: Minimum of three years of professional nursing experience in a clinical care setting preferred.

The RN Health Coach Palliative Care must attend Palliative Care training and obtain certificate of competency or attain palliative care certification at the earliest opportunity, not to exceed 2 years.
